Bequest
Leave a bequest to the Eskenazi Health Foundation through your will.

The following language is recommended when preparing your will:
“I give to the Eskenazi Health Foundation, an Indiana charitable organization, the sum of $____ (or in the case of securities, real estate or personal property such as art or jewelry, please describe) to be used for its general charitable purposes.”

OR

“I give to the Eskenazi Health Foundation, an Indiana charitable organization,  ___% of my estate, to be used for its general charitable purposes.”
Life Insurance Beneficiary Designation

Name the Eskenazi Health Foundation as the recipient of all or part of the proceeds of a life insurance policy that is no longer needed for family protection.

Retirement Account Beneficiary Designation
Name the Eskenazi Health Foundation as a full or partial beneficiary of your retirement plan. Such a designation may save your family from paying income tax on those same funds upon your death.
